Dear Patients and Families:

As you may be aware, the CDC has recently recommended that all nursing home patients to receive a booster dose of the COVID-19 vaccine, and the FDA has also endorsed administration of the Pfizer, Moderna and J&J booster vaccinations through amended emergency use authorizations. We are working in partnership with Giant Food to administer booster doses on November 2, 2021.

Consent forms will be emailed this week to patients or their representatives for electronic signature. Please be aware that the email will be sent from “Docusign”. If we do not have an email address and cell phone number for an individual on file, you may contact me by calling 540-863-4096 or I will be contacting you to provide verbal consent.
